Subject: [PATCH] Allow JuliaInterpreter 0.12

This simply adds compatibility with the latest JI.
Debugger behavior does not change, as it does not use the APIs whose
behavior JuliaInterpreter 0.12 changed (`Frame(mod, ex)` for top-level
expressions and stepping into them with `:s`).
