Revise PR template

The existing pull request template has not been significantly updated in a while, and contains a lot of checklist items.

We should review the existing template, remove any items which are no longer relevant, and potentially move some of the more granular information into another document in the repo.

We should also add a "Demo" section to ensure that every PR includes a demonstration of the work in action.
