What does this PR do? What is the motivation?

• Addresses the feedback given here.
• Adds session-level templates in prompt_templates.md
• Reorders prompt_templates.md to discussion session, trace, and then span.
• Reorders the menu to display Session-Level Evals before Trace-Level Evals
