Update migration guide with Persisted Doc Hash Algorithm and ESM breaking changes

Description

This PR adds two breaking changes to the doc:

• Persisted doc hash algorithm's default is sha256: [client-preset] Make persisted docs default hashing algorithm sha256, and follow recommended format #10652
• Update to all deps to ESM:
  • Update package deps #10650
  • Bump inquirer and improve related tests #10651

Also, apply minor formatting for consistency
